- You MUST submit one form of identification document for EACH family member in a family.
- Accepted forms of Identification: Passport, Driver's license, National ID. Student ID not accepted.
- Only Latin Characters are allowed on this form (Hebrew Letters not accepted)..
- Friends, collegause, etc are NOT to be considered family members and must submit a separate form.
- You will be able to select the Synagogue/venue and dates of cerimony on the next step.
Failure to comply with these instructions, will deny you and your party entrace to the event.
